General awareness about blood diseases

Every drop of blood tells a story of oxygen reaching your muscles, infections being fought off silently, and wounds healing without a second thought. But when something goes wrong in this finely tuned system, the effects can range from mild fatigue to life-threatening illness. Understanding Blood Diseases

Blood diseases are probably the most common diseases in the world. Blood is formed from the spongy bone marrow present within the bones and produces three types of blood cells namely red blood cells which carry oxygen, white blood cells which fight infections and platelets which prevent bleeding. Blood diseases can involve any or all of these blood cells and can be classified into two major categories- benign and malignant blood diseases. In simple words benign diseases are all non-cancerous blood diseases and malignant diseases are blood cancers.

Benign (Non-Cancerous) Blood Diseases

Benign blood diseases are far more common than blood cancers and, in most cases, are manageable with timely diagnosis and treatment. They can affect any of the three blood cell types; red blood cells, white blood cells, or platelets and range from mild, easily corrected deficiencies to more serious inherited or bone marrow-related conditions.   • Iron deficiency anemia – one of the most common diseases worldwide, often caused by worm infestation in children, stomach ulcers and piles in middle-aged or elderly people, and increased menstrual blood loss in females
  • Vitamin B12 deficiency anemia (megaloblastic anemia) – seen in people on a vegetarian diet, or in others with defects in vitamin B12 absorption, termed pernicious anemia
  • Anemias due to red blood cell destruction – including thalassemias, sickle cell anemia, hereditary spherocytosis, immune-mediated blood cell destruction, and anemias triggered by certain drugs or viruses, particularly in people with deficiencies of certain red blood cell proteins.

A few important conditions to note:

Patients taking painkillers and anti-platelet drugs can develop stomach ulcers and bleeding, leading to anemia; this requires evaluation through endoscopy and colonoscopy to locate the bleeding ulcers.

Thalassemia major is an inherited red blood cell defect, passed on when both parents carry thalassemia minor. All pregnant women should be evaluated for thalassemia minor using hemoglobin, MCV, and HPLC tests, since a child born with thalassemia major requires lifelong blood transfusions.

Thrombocytopenia occurs when platelets are destroyed within the body, causing bleeding symptoms due to their reduced numbers.

Aplastic anemia results from bone marrow failing to produce blood cells. It’s a life-threatening condition, since no blood cells are formed; these patients need an urgent bone marrow transplant from a healthy donor to replace the damaged marrow.

Other benign conditions include:

  • Hemophilia – an inherited bleeding disorder
  • Deep vein thrombosis – caused by blood clotting within blood vessels

Malignant Blood Diseases (Blood Cancers)

Malignant blood diseases occur when abnormal blood cells grow uncontrollably, crowding out healthy blood cells and interfering with the body’s normal functions. Unlike benign conditions, these require prompt diagnosis and specialized cancer treatment, though many are highly treatable and even curable with the right approach. Malignant blood disease into three main categories:

Leukemias
Leukemia is a cancer of the blood-forming cells in the bone marrow, causing abnormal white blood cells to multiply rapidly. It can progress very quickly or very slowly, depending on the type.

Leukemias can be:

  • Acute – immediately fatal if not treated right away
  • Chronic – progress slowly and are usually not immediately fatal

Today, leukemias are among the most highly curable cancers. Refractory, relapsed, or high-risk leukemias require bone marrow transplantation for cure.

Lymphomas
Lymphoma is a cancer of the lymphatic system, which plays a key role in fighting infection. It typically develops in the lymph nodes but can spread to other organs if left untreated.

Lymphomas are classified as either Hodgkin or non-Hodgkin lymphoma and are treated with chemotherapy.

Leukemias and lymphomas usually present with:

  • Weakness
  • Breathlessness
  • Fever
  • Bleeding tendencies
  • Enlargement of lymph nodes, spleen, and liver

Diagnosis is confirmed through bone marrow and lymph node biopsies.

Myeloma
Myeloma is a cancer of plasma cells, a type of white blood cell found in the bone marrow that normally helps fight infection. As abnormal plasma cells multiply, they can damage bones and impair kidney function.

Myeloma presents with bone pain and kidney failure, and is treated with chemotherapy followed by an upfront bone marrow transplant.

How Blood Diseases Are Diagnosed ?

A Complete Blood Count (CBC), also called a Hemogram, is the most common initial blood test used to identify most blood diseases. It measures the levels of red blood cells, white blood cells, and platelets, and often gives the first clue toward a diagnosis. Depending on the results, further tests such as peripheral blood smear examination, bone marrow biopsy, and specialized blood tests may be required to confirm the exact type of blood disease and plan the right course of treatment.
